Please complete this FCPS Field Trip Form for Tag Day.

This form is required of all parents and students who MAY drive band students for ANY reason on Tag Day. This covers our bases. Thanks for helping!

Once you've completed and signed all forms, follow the steps below

  • Click on the "Upload" button-it will take you to a corresponding Google Form

  • Make sure the email address at the top of the form is your student's FCPS address (e.g. 1234567@fcpsschools.net). If not, click on the "Switch account" link

  • Answer all applicable questions and submit the Google Form

GRADUATION & ASSESSMENT

MARCHING BAND & WINTER GUARD STUDENTS

Field Trip Forms need to be completed

If you participated in Marching Band or Fall/Winter Guard you only need to complete:
1-Assessment Field Trip Permission Form (all grades)
AND
2-Graduation Field Trip Permission Form (Seniors do not need to complete).

NON-MARCHING & NON-GUARD STUDENTS:

Field Trip Forms AND an Emergency Care Form need to be completed

If you did not participate in Marching Band or Guard, you will need to complete:
1-FCPS Emergency Care Form
2-Assessment Field Trip Permission Form (all grades)
AND 
3-Graduation Field Trip Permission Form (Seniors do not need to complete).
