Please complete this FCPS Field Trip Form for Tag Day.

This form is required of all parents and students who MAY drive band students for ANY reason on Tag Day. This covers our bases. Thanks for helping!

Field Trip Forms need to be completed

If you participated in Marching Band or Fall/Winter Guard you only need to complete:
1-Assessment Field Trip Permission Form (all grades)
AND
2-Graduation Field Trip Permission Form (Seniors do not need to complete).

NON-MARCHING & NON-GUARD STUDENTS:

Field Trip Forms AND an Emergency Care Form need to be completed

If you did not participate in Marching Band or Guard, you will need to complete:
1-FCPS Emergency Care Form
2-Assessment Field Trip Permission Form (all grades)
AND 
3-Graduation Field Trip Permission Form (Seniors do not need to complete).

Print, complete and turn in prior to or at the activity

If you plan to pick your student up from a competition or activity, you must complete BOTH forms for that event. Turn these forms in prior to or at the activity. No student will be released without these forms AND the adult designated on the forms in sight.
